How it works

How the work is organised.

DPRP starts with people, the services around them and the organisations that know the area. Each programme adapts the work to its own community.

Listen and assess

People with disabilities and their families describe the support they need. Trained local workers use assessments to understand needs and identify appropriate care or referrals.

Plan and follow up

Rehabilitation plans set out practical support and follow-up. Depending on the person, this may involve healthcare, useful equipment, school support or help with everyday activities.

Support early care

Care during pregnancy, birth and childhood runs alongside rehabilitation. Health workers help families get advice, recognise concerns and reach appropriate services.

Agree how the work will continue

Local government and partner organisations agree responsibilities, costs and plans for staff and services. They review progress together and prepare for the end of outside funding.
