People with disabilities and their families help identify what gets in the way of daily life. Health workers, teachers and local organisations work with them to plan practical support.
Make support part of local services
DPRP helps connect rehabilitation, preventive healthcare, education and other support through the services already in the community. Local government takes part in planning, funding and reviewing that work.
Prepare for the years ahead
Lasting support depends on people, skills, budgets and clear responsibilities. These are planned from the beginning, so local services can prepare to continue when outside funding ends.
Learn from what happens
The work needs regular review: whether people can reach services, whether support helps in daily life, and whether local teams have the resources to continue. The answers help each programme decide what to improve.
